Diabetes Mobile Clinic (DMC)

​The DMC operations are overseen by an interdisciplinary team from the Alex Community Health Centre Mobile Health Program and the Calgary Diabetes Advocacy Committee.

We understand that when we are treating diabetes, we are not treating one organ, rather a whole person that lives within complex social systems. Knowing this, we have developed a flexible, welcoming, and holistic menu of services to support those who may struggle to access traditional or mainstream diabetes care.

Healthcare services
Individuals can access the DMC at various locations across Calgary during pre-scheduled appointment or on a drop by basis. Their initial assessment will include the following care services offered on the DMC:

1) A comprehensive suite of point-of-care screening tests for diabetes microvascular complications, including:
  • glycated hemoglobin (A1C) testing
  • eye (retinal) screening
  • neurovascular examination of the lower extremities, and
  • testing for albuminuria to detect diabetic kidney disease.
Basic foot care will also be provided longitudinally for those who need it.

2) Individual diabetes self-management education that can include dietary counselling (tailored to context), as well as basic diabetes education and detailed insulin titration and glycemic management advice.

3) Comprehensive endocrinology care to optimize medical therapy and treatment for diabetes, as well as other cardiovascular risk factors like hypertension and cholesterol and assessment for other health conditions like obesity and cardiovascular disease.
DMC patients and the healthcare team will collaboratively determine subsequent follow ups.

Based on the patient’s eye, kidney, and feet screening results, patients may also receive trauma-informed referrals to kidney, neurovascular, and eye specialists.

Who's on the bus?

Diabetes educator
Provides self-management education, which can include medication and dietary counselling.
Licensed practical nurses 
Completes point-of-care screening testing and basic foot care. 
Peer support liaison 
Greets patients, creates appointment and sends reminders about upcoming appointments. 
Endocrinologist
Follows and adjusts patients medical therapy and treatment for diabetes and cardiovascular risk factors.
